Membership-Based Primary Care

What Membership Covers

Membership includes comprehensive, relationship-based adult primary care, with a focus on both current concerns and long-term health.

    • Unlimited primary care visits

    • Extended appointment times

      • Up to 2-hour new patient visits

      • 60-minute follow-up visits

    • Same-day or next-day visits when medically appropriate

    • In-person and virtual visits

    • Direct access via phone and patient portal

    • After-hours availability for urgent concerns

    • Coordination with outside clinicians and specialists

    • Assistance obtaining and reviewing outside medical records

    • Interpretation of labs and imaging performed elsewhere

    • Coordination with hospital teams if you are admitted

    • Evaluation of fitness, sleep, nutrition, metabolic health, and emotional well-being, when relevant

    • Personalized health planning

    • Structured follow-up and accountability over time

    • Optional group visits or virtual office hours

    • Educational materials and guides

    • Discounts on select programs (e.g., sleep optimization)

    • Case-by-case home or facility visits may be available when clinically appropriate and schedules allow.

What Membership Does Not Cover

To keep fees predictable and transparent, some services are not included.

    • Vaccines (obtain these via your local pharmacy)

    • Pap smears or gynecologic exams

    • Joint or trigger-point injections

    • In-office diagnostic testing (e.g., flu, strep, RSV, urine dipstick testing)

    • Cannabis certification

  • These are often covered by insurance:

    • Laboratory processing fees

    • Imaging studies

    • Medications

    • Emergency or hospital care

    • Specialist visits

    • Medical devices or supplies

    • Personal training or nutrition services

    • Extensive forms, letters, or documentation may incur an additional fee (you’ll be notified in advance)

Membership is not insurance and does not replace health insurance.

  • Weight Management

    Intended for adults who want a structured, sustainable approach to weight loss grounded in evidence and long-term health. Includes: group sessions, guided check-ins, and accountability components to support consistency and follow-through over time.

    * Medical evaluation, diagnosis, and prescribing occur separately within the context of a primary care relationship when indicated.

  • Sleep Optimization

    Designed for adults seeking a practical, structured approach to improving sleep through behavioral strategies, habit formation, and accountability.

    Areas of Focus: common contributors to poor sleep, screening for sleep disorders, circadian rhythm alignment, behavioral and lifestyle strategies, cognitive approaches to sleep-related anxiety, tools to support consistency and recovery

    *If a sleep study is clinically indicated, I will refer you to an appropriate testing source. Sleep studies are not performed in-office.
